Timofeyev (Malcolm McDowell) is a patient in a mental hospital; he thinks himself to be a reincarnation of Yakov Yurovsky, Russian revolutionary who was the chief executioner of Emperor Nicholas II and his family in 1918. Dr. Smirnov (Oleg Yankovskiy) spends much time, talking with Timofeyev, who tells him the story of the event of 1918 through the eyes of Yurovsky.
