The gun is customized with a rear pistol grip, a leather strap on the foregrip, a custom Royal Arms 12 point breaching muzzle and a side shell holder. A cultist is also seen with a regular Remington 870 shotgun during the church shootout. Some state troopers are also seen with regular Remington 870s at a roadblock.

Judging from the typical dark grey finish, old safety lever and linear barrel, the gun is a genuine M1911A1 Army version, not a more common Series 70 often used to impersonate the gun. Milton's original handgun was supposedly going to be a .357 Magnum revolver. After the "God Killer" was made, Milton's sidearm was changed to an M1911A1 , to be less similar. The gun has two visible jam, exaclty when Piper shoots the cop and then when Milton reloads after shooting a cultist woman driving a pick-up.
