As the wheel turns William decides who lives or dies by either enduring pain himself to save two of his choice or not doing anything for the other four and when the person stops in front of the shotgun if William chooses to not do anything, the mechanism the shotgun is rigged to, aimed at them cycles the action and fires at them; killing them.

Unknown Pistol
Upon hearing a disturbance in his office building late at night at seeing the shadow of a man with a gun, William (Peter Outerbridge) retrieves an unknown pistol from a drawer, He uses it to shoot the man which turns out to be a security guard just before he is ambushed from behind by the real assailant. The pistol looked to be a Glock of some model but it was dark. Anyone got a real ID? I was thinking Glock 17 or 22.
